Dakota Rehabilitation Center has been providing outpatient rehab services in Sioux Falls for over 10 years. The clinic was founded with the goal of excellent care, exceptional customer service and a friendly, comfortable atmosphere. Our staff is dedicated to helping you with a variety of conditions. We want you to feel better and get back to those activities you long to do again. From assisting you with insurance to helping you stay on track once your therapy has ended, we want to be partners on your road to recovery.
Michael Deckert, Physical Therapist
Mike grew up in a small town in SD. He graduated from Sioux Falls College with a BS in Biology and continued his education at Wichita State University in Wichita, Kansas, where he graduated with a BS in Physical Therapy. Mike has 19 years of PT experience, specializing in outpatient treatment of neuromusculoskeletal conditions. Mike is a co-owner of Dakota Rehab Center.

Certified Case Manager (CCM) & Certified Disability Management Specialist (CDMS). Deb grew up in Sioux Falls and graduated with a BA from South Dakota State University. She has 20 years of case management experience, working in an outpatient rehab setting for the past 17 years. Deb is involved in program coordination, HR and marketing. Deb is a co-owner of Dakota Rehab Center.

Lori was born and raised in South Dakota. Her family moved to Virginia in 1996 and returned in 2005. Lori has been in the healthcare arena for 12 years. She has worked both in clinical and office areas, working as administrative and executive assistant positions.

Kristin grew up in Sioux Falls and attended the University of South Dakota to obtain her Physical Therapist degree. She has been in practice since 1998 and in the outpatient setting since 2000. She enjoys working with a variety of diagnoses.

Leslie Bouma, Occupational Therapist
Leslie graduated from The University of Kansas in 1985 with a Bachelor’s degree in Occupational Therapy. She has a diverse background including: neuro rehab, pain management and hand therapy. Currently specializes in pre and post surgical upper extremity conditions, repetitive use disorders, work conditioning/ hardening and ergonomics.

Andrea graduated in 1996 with a Master of Physical Therapy degree from The College of St. Catherine’s in St. Paul, Minnesota. She also attended Augustana College where she graduated with a Fitness Science degree. Andrea has over 12 years of experience as a Physical Therapist and enjoys working with all patient types.
